Wellness Elite Fitness, a service offering of Wellness-Corporation LLC (collectively, “WEF,” “we,” “us,” or “our”), respects your privacy and is committed to protecting the personal information you share with us. 1. Information We Collect
We collect information in three ways: information you provide directly, information collected automatically, and information received from third parties.
• Information you provide directly: name, email, phone number, mailing address, date of birth, emergency contact, payment information, membership preferences, health-intake responses (including medical history, goals, and contraindications), consent forms, photographs (where applicable), and any content you submit through contact forms, free-day-pass requests, the AI kiosk, or direct communication with staff.
• Information collected automatically: IP address, device and browser type, referring URLs, pages viewed, time on site, and interaction data collected via cookies, pixels, and similar technologies including Google Analytics, Google reCAPTCHA, and Meta Pixel.
• Information from third parties: membership-management data from GymMaster, payment-confirmation data from payment processors, and limited conversational data from AI-model providers used to power our kiosk (including Anthropic).
2. Health and Wellness Information
WEF is a wellness and fitness facility operating under the medical direction of our Chief Medical Officer. Certain services — including medical weight loss, IV infusions, cosmetic injections, and physician consultations — may generate information that is treated as protected health information under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). When HIPAA applies, your information is handled in accordance with our separate Notice of Privacy Practices, which governs over this Privacy Policy to the extent of any conflict. Non-medical wellness data (e.g., gym usage, recovery bookings, intake forms for non-medical services) is not HIPAA-regulated but is still treated with care under this Policy.

We use the information we collect to:
• Provide, personalize, and improve our services, memberships, and facility operations;
• Schedule appointments, process payments, and manage member accounts;
• Respond to inquiries, contact-form submissions, and AI-kiosk conversations;

• Serve and measure advertising on platforms including Meta (Facebook/Instagram) and Google;

4. Cookies, Analytics, and Tracking Technologies
Our Site uses cookies and similar technologies to support functionality and measure performance. Opting out of marketing will not prevent transactional messages related to your membership or appointments.
6. AI Concierge Kiosk
Our on-site AI concierge is powered by large language model technology provided by Anthropic and other vendors. Conversations with the kiosk may be transmitted to and processed by these vendors for the sole purpose of generating responses. We do not instruct the kiosk to request or record protected health information. You should not enter sensitive personal data, payment information, or confidential medical details into the kiosk. Limited conversation transcripts may be retained by WEF for service-quality review and may be retained by AI vendors in accordance with their own data-use policies.
7. How We Share Information
We do not sell your personal information. We share information only with:
• Service providers acting on our behalf (payment processors, membership-management software, analytics vendors, AI-model providers, email/SMS platforms, hosting providers), subject to appropriate confidentiality and data-protection obligations;
• Professional advisors (attorneys, accountants, insurers) where reasonably necessary;
• Regulatory, governmental, and law-enforcement bodies where required by law or legal process;
• Successors in the event of a merger, acquisition, financing, or sale of assets, in which case personal information may be transferred as part of the transaction.
